Xbox One Controller Disconnecting, re-connects after a lot of effort, then disconnects again..

Alright so I'm posting this on the Thursday evening after the Forza Horizon 2 and Mordor release. I have never before had an issue with either my controller or my xbox one. Last night, I was playing and my controller notified me it was about to die. I have
the Play and charge kit so I plugged it in. As soon as I did so, the controller went off. When I turned it back on, It wasn't connected to my xbox. I tried to reconnect without success.Then I unplugged everything, pulled out the battery, and shut off the xbox.
I then plugged the controller back in and let it charge over night. I fired it up this morning to see if it was working, and it was. Without issue. I played for a bit and then turned it off. A few hours later I went back to it and fired it up and I couldn't
connect again. Did a hard-reboot on the xbox, pulled the controller apart again, and it worked again. My friends mentioned I should have done a controller update some time ago, Plugged in the controller to update it, and as soon as the update completed, it
shut off. Again. I could not get it to connect. I installed both the Lord of the Rings: Mordor and Forza Horizon 2 from a disc tuesday. What is going on? Can anyone help me? I'm really starting to get frustrated...

Xbox One controller issue

Yersterday i was playing Xbox One with my controller connected (wired), with a battery (it's not the Play and Charge). I used to play like this for months, and until now i have had no problems with this.
After 2 or 3 hours playing, my controller turned off and it wasn't connecting, so i disconnected the cable and connected again, turning on the controller. 5-10 seconds after that i felt a burning smell coming out the controller, so i disconnected the cable again.
i tried to sync it again with batteries ( i tried with 4 pairs of them, fully charged) but it blinks twice and turns off.
It's very funny because the controller still works with batteries and plugged at the same time, but not wireless or just wired without the batteries, so i thought that could be a problem with the plug and the wireless(the "power" comes from the AA batteries and the connection from the cable).
Is there anything i can do to fix my controller?
PS.: I'm really sorry for my english, i'm from Brazil.

 

Xbox One Wireless Controller Won't Stay Connected In Windows 10

I'm having a problem with the Xbox One Wireless Controller not staying connected in Windows 10.

I'm using one of the newer controllers with the stereo input along with the USB wireless adapter dongle.

Whether after batteries run out, after it disconnects from being idle, or one if it's random disconnects, it refuses to connect again. Pressing and holding the Xbox button causes it to connect for one second then disconnect. It just won't stay connected.
Re-sycning with the dongle will not connect it. Connected with a USB cable, then unplugging will not keep it connected. The only way to use the controller is to restart the computer. Which means I'm restarting my computer multiple times a day.

I've updated the controller firmware and have uninstalled and reinstalled the drive for the dongle. Neither have helped.

 

Yeah im getting the same thing. I'd change batteries or something, then it wouldnt connect when i turned it back on, but the Home bottom was lit. Getting the same thing with my headset too, Id unplug it then plug it back in and get no sound at all

Only way i can fix both of these things is to hard restart the xbox.

I'm getting similar problams with forza2 in mid game controller disconnects brings up message your controller has dc but it's still on and screen freezes have to do hard reset to get it working again
