Xbox One Continuous App Freezing and Closing

For a while now, I have been experiencing issues on my Xbox One (DayOne Edition). I have tried several things and nothing seems to help. I have followed steps to powercycle/hard reset it. Uninstall and reinstall. I even went through and deleted several apps
and games to free up space to make sure there was plenty of room for whatever the Xbox does in the background. Here are aa couple of the things that are going on:

1. Games freezing and closing back to home

2. Pressing Xbox button while in game to go home with no response and have to powercycle to get it back to home

For the number one item, it mostly happens during call of duty games. I do mostly play call of duty, for example Advanced Warfare, Ghosts, and Blacks Ops 3 Beta. Most of the time it would freezing and make a repeating digital sound and then after about 30
sec it would close down and go back home. But with Black Ops 3 Beta it would do it almost every time I launched the game. Now I understand that it's a beta and not full version of game, but I have talk to several friends and watch videos on YouTube and live
streams on Twitch and YouTube and no one has seem to have this problem that I have come across. I went through the Activision troubleshooting and one of the steps said my NAT needed to be open. Xbox settings show open but in game shows moderate. So I don't
know if that was a game glitch or what but I would to fix whatever it is so I can move on from this.

For item number two, hitting the Xbox button while in game to go home with no response. I have only come across this in game and not in app. Again during call of duty advanced Warfare and black ops 3 Beta. I can double press for snap menu and it pops up
just fine. I can also press hold to turn off console or controller. But just pressing once to go back home doesn't do anything. This is intermittent but has happened numerous times.

I would like to add I am a preview program member and I have Time Warner Cable internet just in case those details are needed for whatever reason.

Please any help would be appreciated. I want to get it straightened out before launch of BO3 so trying now. Thanks in advance.

Accessories App Malfunction

My son installed the XBOX accessories app this week on his XBOX one. But since then we get a continuous pop up regarding the controller and button mapping. It has also meant nothing can be selected in the game.

Can anyone help please?
