XBOX Controller issue

So, I've been playing NHL15 recently because I always wait until I buy the next game.

Anyway, when I play, two "select a side" controllers show up, yet I only have one controller. I nearly always stream to my PC with a wired connection, but my wire broke so I wirelessly connected it to my XBOX. I booted up NHL15 and that's when the error
occurred. I disconnected my XBOX from the power cord, stopped the stream, re-synced my controller to the XBOX and nothing works. This is a problem because I cannot play "Be a Pro" with this error.

Hello everyone, ever since I bought my new controller to use with my new turtle beaches, the controller sometimes disconnects and the guide button flashes when I record gameclips. It also makes my turtle beach headset crackle and pop. It also crackles and
pops when there is a loud noise in the game. I know it isn't an issue with my headset because my headset works fine on my computer and phone. But the controller has been like this ever since I got it brand new out of the box. Is this normal? Does anyone else
experience this? The problem is usually fixed when I reinsert the batteries and reinsert the headset jack, but it comes back again when I record more gameclips and when more loud noises happen.
