Every console comes with a free month for up to 3 gamertags. It's only available during the account creation process by choosing yes to Join Live and then choosing Upgrade Later when it gives you the gold options. If the trial is available, it automatically
gives it to you.

During the account creation process (and as vote said it has to be done on the console) you have to choose yes when it asks if you want to join Live, then choose upgrade later when it gives you the options for purchasing gold. Then it will automatically
give you the free month if it's available.

the microsoft account automatically gets an xbox account made for it on creation. when you log into xbox.com with your microsoft account and password you will see it and get to change the name on the gamertag.
