If changing the picture via Xbox app keeps not working, you can try waiting for the next update and try again once the proper team release it for iOS or Android in case you can test with an Android phone

Is this happening with an specific image?
Could you try using another to see how it goes?

Also, in case you didn't try it, change your Xbox profile image via Xbox mobile app.
Sometimes that does the trick
