Xbox Adapter Problems

Whenever I start up my Xbox one with my adapter and headset plugged in, as soon as i press any button my xbox freezes and I am forced to shut it off. But when I start the Xbox without it plugged in it works fine, afterwards I plug in my adapter and headset
it takes about 10 seconds for me to select something different on the dashboard, and when I finally get a game started there is no audio for it. I can hear the beeps it makes when I turn the volume up all the way and all that but not the actual game audio.
My headset worked fine before there was a update with the store and all that. I tried updating my controller a second time through the settings>system thing but the xbox freezes when the update stops.

Xbox wont start up.

Xbox won't startup. Dc adapter is fine, looks like motherboard problem. No video, no sounds...

No. It might be because of the new update for the month, I think it came out early. If that;'s the case I will unregister from that ***. D:

 

Actually, I just tried playing without the headset and using the stereo, and I just noticed no sound is coming from that too. The xbox 360 works fine though.

 

Nevermind, the xbox controller is still on after I turn the xbox off now, and is impossible to select anything although is synced and worked fine yesterday. I just got watch dogs from midnight release and it's a shame I can't play it.

 

got the same problem after updating my controller anyway gonna ask a new headset
