Xbox Achievements glitched

To be clear before I go into this I am ONLINE and HAVE XBOX LIVE and a GOOD INTERNET CONNECTION.

Recently I was playing two games, one was Tom Clancy's the division and the other game was Call of Duty Modern Warfare Remastered.

It started on Modern Warfare Remastered when I was beating the whole game on veteran and whenever I was on the mission safe house and this happened through the rest of the game as well and also cheats were not on when I was playing. After me and my dad called
Xbox Support about messed up Privacy settings and the achievement glitch I thought it was fixed but I did not attempt at the Veteran play through on COD again. So about two weeks later on November 29, 2016 while I was playing the survival DLC on the division
I finally beat survival on the division but when I got back to the Terminal and walked out I did not get the achievement so me and my dad contacted Xbox support and they said that they couldn't do anything about it and that I would have to contact a developer
or post on the forums so I chose the forums.

And If anyone sees this I would appreciate it if you spread the word or if its a dev please fix this it would mean a lot and also just message me to contact me for my Gtag it would help because I am getting a lot of games for Christmas and Would like to
reach 30,000 Gamer score (Also i'm new to the forums)

TWD survival instinct achievement question

Some achievements are glitched like that. Just be glad it wasn't glitched the other way, where you've distracted several hundred zombies and still no achievement

 

Halo 4 glitched achievements

This is currently a big issue with Halo 4 that has not been addressed by 343 despite several playlist updates.

Several kill based achievements are glitched in Halo 4 multiplayer. Fortunately, only 1 achievement is glitched for me, but it is the final one I need, and I know there are several players dealing with similar issues with several achievements.

The achievement I have glitched is Ashes to Ashes. Is there any known way to get these glitched achievements?

 

My Achievements had been working find up until 2 weeks ago. Had trouble unlocking the 'Mightier than the Shovel' BF1 achievement. Had to do it twice and power cycle my Xbox along with clear the cache to get it to pop upon after trying it a second time.
Just started playing Dark Souls 3 last night and beat the Dragon slayer Armour. No achievement unlock popped for me. At this point I have no idea if it's related to my Xbox One console or if it's something else entirely. I have a good internet connection.
All the required network ports are open, along with my NAT type being open. Never had this issue before until now. I'm usually get the new dashboard updates in the insider hub program so I'm wondering if it's the O/S that's messed up.
