Xbox 8 strike system sucks and should seriously be looked at

My friend's account is getting mass reported, has been banned for a year, is getting mass reported again, and now has 18/8 strikes. What can he do, and why does the Xbox enforcement system stink at detecting fake reports? This has happened in the time span of 2 days. He changed his picture to an official picture the second he was banned. This was also a result of the Xbox app not warning him properly that his gamer picture was not appropriate. In fact, I also tried to change my gamer picture to something not at all inappropriate, and it has yet to change. Don't release broken trash. Also, please keep in mind that you can find homophobic, racist, and horrible profile pics that never get banned for weeks.

my friend got reported for game icon even though he took it down, how does this make sense he only had it up for 2 minutes and took it down next thing we know he has 18 reports out of 8

Perhaps, you should ask yourself why you have 3 strikes and whether you need to modify your behaviour while online to stop getting strikes against your account.

That should be patched. C4 are pretty overpowered for it to do that.
