    I have a "Day One" controller that has given me no problems until I purchased and installed headset adapter.

    I followed the directions to the letter.

    I removed battery, plugged in adapter and my Turtlebeach X41 to the adapter and updated the controller.

    Re-installed battery and began playing as I normally do happy that I can now chat using my TB X41s.

    For no reason my controller disconnects and I get a message on screen to reconnect controller.

    Problem is that its not that simple. You just cant press the Xbox button because its locked up and does no good. Pressing the small black reconnect button on the front of the controller with button on the side of the console does no good either. Removing
    adapter does nothing. The controller freezes during game play for no reason.

    I have the Xbox battery rechargeable battery pack that I must remove then plug in the cord to get the controller to reconnect, then after a few min, I can re install the battery pack and unplug the cord.

    The battery pack is fully charged so that's not the problem, the controller has been malfunctioning since the update.

    If at all possible I would first try disconnecting and reconnecting the microphone a few times just to make sure it is securely connected, After this I would then check to see if the

    controller requires a firmware update

    XB1 controller disconnecting after adapter update.

    Having the same problem as War Chicken 619. Have a Day One controller, that has been working fine until I purchased the MS headphones with the chat adapter. After doing the required update my controller will randomly disconnect with the only way to resolve
    the problem is to remove the battery. Sometimes I will get a warning the controller is going to disconnect with a slight buzzing in game audio and if I point the controller directly at the Xbox unit it will stay connected, but that does not work all the time.
    In an hour of game play I suffered 3 disconnects and 4 almost disconnects. This is very frustrating and hoping this is a issue that is being addressed.

    Many people have had this problem. I've seen around half a dozen other threads about it.

    My controller has been "crashing" and disconnecting since I applied the controller firmware update when I got my Official Stereo Headset and Official Stereo Headset Adaptor on Friday. My Xbox One was in the Preview Programme, and I had already previously
    updated the controller during that, yet had to update it again when I got the headset and adaptor on Friday.

    I can play Titanfall for around two hours with the adaptor and headset connected, and then I'll get an awful noise through the headset and the Xbox One will tell me the controller has been disconnected even though the light on it is still on.

    I have to take the battery out of the controller and put it back in to be able to get it to come back on and reconnect.

    I have tried going to Advanced Network settings and selecting to "Clear" the "Alternate MAC address" as mentioned in a post on here somewhere, and have also tried power cycling the console. Neither has helped.

    Xbox Support on Twitter first claimed it didn't sound related to the controller update, even though it never happened before the controller firmware update and even though I provided them with links for various threads about it here at Xbox.com along with
    Tweets from The Verge's Tom Warren who's suffering the same problem.

    They now suggest cutting power at the wall for 5 minutes and re-syncing, which tbh I doubt will be any help at all.

    Agreed. I highly doubt that unplugging the unit from the wall and re-syncing the controller is going to solve the problem, but I will try it. Not many other options at this point. Have seen the other "fixes" that Tezcatlipoca listed and seem they all fail
    at solving anything. I really believe that this is a failed update to the controller. I never had this problem at all before the update, and usually when something goes wrong you go back to the last thing you changed, the controller update. I wish MS would
    address this issue.

    If your Xbox one Controller doesn't work or gets disconnected after you plug in your Stereo Headset Adapter, unplug your adapter,
    and then update your controller without the adapter. After the controller is updated, insert your Stereo Headset Adapter back into the controller.

    Exactly as Tezcatlipoca described.

    When I initially got the XB1 I played the crap out of BF4 with no issues. Even titanfall didn't give me an issue until I purchased the chat adapter for my Tritons and had to update my controllers.

    I have 2 controllers and both do the same thing after updating them. Awful noise through the headset and the system tells me it's disconnected even though the controller light is still on.
