May I ask what device you're using? This is to check what necessary steps we have to perform.

the game starts without a problem .. loads and when I enter the game mode sometimes closes at the moment but other time I can still play 5min

I will be playing this game until the servers are closed (whenever this happends), so I am not worried about how much time the servers have left before they get cut off

1) I want to know that too, if windows games counts or just games played in the xbox

2) If you go to the game hub and go to stats it will show you how many hours you played that game, at the beginning of the month write on a paper how many hours you played that game, then you need 5 more hours, easy way to track it.

3) Mos of the time you will be able to see that stamp after 90 - 105 hours

Great that you can now get beyond the 50th stamp card , not sure how long this has been possible but got to the 51rst stamp card today and the xboxachievements.com site for xbox Fitness indicates new achievements available for the 60th 70th 80th 90th and
100th stamp card -not sure whether this is automatic or as a DLC - My motivation is back !
