How do you fix error code 0x89234013?

If its a Dell you should actually have a Dell Update app you can use in your start menu that can automatically tell you when there is an update available. If its not set up for automatic checks just open it and run a manual check for updates.

    Unexplained bandwidth usage

In Settings, there's one setting called "Keep my games and apps updated" and if that is checked it is very plausible that, like 2Bad4You says, there were game updates happening. 68GB is entirely reasonable for a few game updates these days. You can uncheck
that to stop the console from performing updates automatically in future without your knowing. Without automatic updates you'd be forced to do start them manually, or have a pop-up when you start a game saying "there's an update available" that prevents you
loading the game until the update is complete. But if data caps are an issue, then updating games as and when you want to play them could be preferable to having them be done automatically in the background.

    Black ops 2

There may have been a problem when installing the update and could have corrupted files. Try deleting the title update and then launch the game and it will automatically install the update again. ;)

I can't find the update for cyberpunk that I heard about

If there was an update, then depending on the console settings, it would either download automatically, be available in the update section in your games and apps, or tell you there was an update when you started the game. Chances are you may already have
    it.

I keep losing my Xbox Live connection when i start any games including apps/games, PLEASE HELP

This sounds like a corrupted update cache on your console.

Each Xbox 360 console has a special dedicated space on it's storage drives, where it will store game title updates from the last 5 games you played.

Unfortunately, after some time it can be that that storage area gets corrupted or "too cluttered", and game updates cause problems when you start a game.

When a game is started, it checks for possible updates. If there is an update, you get disconnected from Live and the game tries to update.

If the update cache is corrupted, you get disconnected EVERY TIME you start a game.

To resolve that, go to the dashboard, and go all the way to the right to the settings hub.

There go to the system tile in the upper left. Then go to the stroage settings.

Highlight any storage device and press the Y button.

    Choose to "Clear System Cache".

Repeat that process 3 times for good measure.

    Now start your game again.

If the game requires an update, you will get signed off again. THIS IS NORMAL!

After that game was updated, you automatically sign in again. If any time later you start the game again, it should start without signing you out.

    But remember:

The Xbox can only store up to 5 game updates.

If you insert a sixth game that requires an update, the least recently played game update will be deleted, and the new game update will be applied.

If you now insert the old game again, it will again sign you out, update itself, and delte the second oldest game update.

So if you "juggle" six games with their updates, it might still be that you get signed off every time you start a game, if that game got it's update deleted previously.

    Wondering what this means....

I think it means the game has updates waiting, I have my console set to slow start, so it does not update automatically, and when I have those blue asterisks in the white triangles the game updates when I boot it up.
