    I was billed 3 times for the upgrade to Premium and have tried contacting support to never get an answer

    Free 3 month subscription to Xbox live?

    You can add the 3 months to your account. The next time your service is to be billed, those 3 months will be used instead and after the 3 months you will be billed for your normal service.

    This is according to what I've done.

    Premium? meaning for what game or service? If you are referring to gold it possibly stacked meaning applied multi times, but for games like bf4 premium is only needed once. chat support can assist with multi charges &/or contacting US phone support at 1-800-469-9269.
    I would highly suggest having the order numbers available for the account. which can be found at Sign in to your Microsoft account or on xbox one settings > account > payment & billing > order history.

    Try sign in to your microsoft account and check your payments to see if you can get a refund
