I have been trying for weeks to get Microsoft to replace a controller that is under warranty. I have provided my receipt and all information requested. I continue to have to jump through the horrible website loops to try and speak with anyone. Is there
not a customer service phone number?

I second the Device Support site. I've not had any issues from there for the repairs I've needed when it came to my 360 and Surface Pro repairs needed.

Is this a controller that was purchased separately or did it come with a console?

You can't register regular controllers, just the Elite, I believe. If you're looking to repair a controller on the Device Support website it would be under Console > Accessories for your selection when creating the ticket.

Xbox Accessories such as controllers have a 90 day warranty from the date of purchase. If you need to send your in for repair you can set this up at
https://devicesupport.microsoft.com. Please be sure to register the controller with your console using the S/N in the battery case

it needs to be registered to your account before you can request a repair/replacement under warranty sign in
