Warframe Awards from "Freedom Rewards" event.

I did the prerequisite, which was complete five achievements and still no reward. (No redeem reward button next to offer as well)

The link to the code redemption page can be accessed from the FREEdom Rewards mission page, in the Warframe section.

Activity takes 96 hours to be processed by our program. Since your fifth achievement since the offer began on the 15th occurred on the 27th, you should see a redemption button appear in the next day or so.
