    Is it mandatory to have my XBL headset plugged in while doing this? I've updated them before with the headset plugged in. However on these recent controller updates I completely forgot about it.

    controller software update?

    It is just a firmware (software) update to improve/add functionality or patch issues the controller might have. Updating a controller is really not different than updating any other device.

    The controller will have an update occasionally.

    Updating controller

    I don't think it is. Only when there is an update for the audio is it required so it reccognizes the headset adapter. But I can't even update my controller at all. Tried 15 times so far on different days and it keeps saying there is a problem. I power cycled,
    disconnected everything, changed usb cords, and even pulled batteries and nothing works. Next stop is calling MS about this. I gave feedback for the preview program but no response. Anyone know of any other fixes??

    Agreed. It keeps telling me my controller is not connected via usb, but it is. Tried taking the batteries out and everything, but it still won't update.

    I have the TB 500X fully wireless so nothing is connected to my controllers, the latest firmware is 1.2.2236.0

    I have the same issue with not being able to update my controller. It kept saying the USB was not plugged in so I tried a USB cable I got from my turtle beach headsets that was for updating the controller. This time it started to work showing the download
    process at 0% but it stopped and said there was a problem. Now my controller is completely dead. It won't even turn on. I'm not going to hassle with MS I'm just going to exchange it.

    Updating controller

    I know what the latest firmware is, I just can't get it. It keeps saying there is a problem but won't tell me what it is. I can't update my controller to that version regardless of what is or is not connected to the controller.

    how do you update your controller? my stock mic wont let me talk but i can hear fine. i feel like this could be the problem

    go to settings, devices, controller, update, make sure its plugged in with a usb cable though.

    Update XBox controller support has steps to update the controller firmware.
    It did not say "Start update" or the update is available for xbox one controller.

    Controller Update for Stereo Headset Adapter Fails

    It sounds like the controller update also updates the headset adapter as the instructions state plug in headset/adapter before updating . So if you have updated the controller with out the headset adapter plugged in its possible that the console will think
    the controller is updated but not recognize that the adapter isnt and then fail or say that the controller is upto date . The only way around this maybe to get a new controller or a controller that hasnt been updated and try the update again . Or try the
    headset adapter in a friends controller that has had the update done and see if it works
