    Headphones

If your headphones have a 3.5 mm jack and your controller supports it then simply plug the headphones into the hole in the front of the controller. If the controller does not have the jack then you'll need to purchase a headphone adapter for the controller.

    Xbox Stereo Headset Question

If I were to buy an Xbox One controller with a 3.5 MM jack, can I plug in my stereo headphones to chat AND hear audio, or do I absolutley need an adapter? TY in advance!

Unfortunately this sounds like a hardware fault and there is nothing you can do software wise to resolve this issue, sorry.

Xbox One Controller thinking LT is being pressed

No, this is an old Xbox One controller without the 3.5 mm jack for headphones. It's a shame that I will have to buy a new controller, but thanks for the help!

Cannot hear audio when headphones connected to my Xbox one controller

Yes ma'am, I tried another controller and my headphones work just fine. I am thinking it has something to do with the 3.5 mm jack... That is my best guess.
