Unable to open any games or apps. One updating game permanently stuck at 4%.

I'll recommend you do a power cycle to clear the system cache

- Turn off your console by pressing the Xbox button on the front of the console for about 10 seconds, until it shuts down completely.

- Unplug the console's power cable. Wait 2 minutes.

- Plug the console power cable back in. Press the Xbox button on your console to turn it on and check if it's all good.

If that doesn't help, I'll suggest you try doing a factory reset keeping games and apps on your console. This will refresh the console and remove all potential bugs without deleting any game data.

- Press the Xbox button to open the guide.

- Select Profile & system > Settings > System > Console info.

- Select Reset console.

- Choose Keep Games and Apps

Once that is done, you'll need to sign into the console using your Microsoft account credentials. If the issue is with your console, that should fix it.

Are you installing the game to an external drive? If yes, can you unplug your external hard drive. Then do this

- Go to Settings > System > Storage.

- On the Manage storage devices screen, select: Clear local saved games.

- Restart your console

Afterwards try installing the game to the internal drive. Don't plug the external drive yet. If you don't have space on the internal, move some games to the external first before you begin.

Have you been reporting this problem through the preview program when it happens? By reporting the problem through the preview program app on your console, it sends data to the team that can be used to fix the problem. Next time it happens, hold down the
power button on the controller (as if you were going to turn off the console) and you should see an option "Report Problem", hit that and fill in the relevant details and send it through. Whilst it won't fix your problem, it will assist the team in being able
to fix it.

Regarding the actual issue, have your tried using another controller? It could be your controller that is bugging out, and not the console? Just a suggestion, as I've not heard of this issue before

I haven't played yet. Just got it and still in plastic until I have time. But bugs are natural in games this large. There are a few in Oblivion still after all these years that can cause quests to be unable to be finished. Also Fallout 3 an annoying
one where people can get stuck permanently in the republic of dave making it unable to finish that voting quest (let one vote then wait, is the solution).

Just have to deal with it if you want to play an open world game.
