The Xbox, account, and game must all be from the same region or you will be unable to download the game. If all of this doesn't match, you won't be able to download the game.

A week on, still unable to download Xbox Game Studios games - Gears Tactics or Ori and Will of the Wisp. Games from other studios download and play just fine...
