two game icons and game downloaded from disk will not play

I attempted to update fallout 4 and it didn't work so I uninstalled the game in hopes to try again. Instead an icon for the game was left and cannot be deleted at all. When I put the game in to download another icon appears and after the installation is
complete the game says to install it again because it cannot be read or makes me go to the store if anyone has an answer to this I would like to know.

Separate games that need a disk from those that I bought online

Can we make some sort of distinction from the games I need to put the disk from the games I bought on my Xbox and can play without a disk in! I have too many games and can't remember witch ones I bought at the store from the games I downloaded! A icon or
marker would be nice to tell me what game I own the disk or what game I can just click and play! Even giving me the options to separate my games from those I bought online from those I have to put the disk in. This would be a huge help to me!

If you have a game disk, insert it now...

I have several games on disk that have been loaded onto the hard drive however, two games (Forza 5 and COD Advanced Warfare) will not load/play from the hard drive.

Whenever I try to play these games, I get the following message: "If you have a game disk, please insert it now". So, the only way I can play these two games (while signed in), is to actually insert the game disks.

I'm not sure why some games will play without the disk and these other two games require the disk to be installed.

Any thoughts?

You aren't getting an answer man, installing from disc (that we pay 60$ for) is an issue they don't seem concerned with yet. It's gonna take one of those class-action lawsuits before they acknowledge there's a problem.

thanks for helping me I've already done all those steps always reaches 78% and disappears from active downloads does not appear in my games and hard disk the yellow icon damaged with the name of the
game download another game and this if I could download it by what the account has no problems that should do

I have downloaded the games and from the 5 games only 2 appeared in my games dashboard. The other 3 did not. So I looked at the disk storage and the 3 games had "!" as icons.

Have deleted them, cleared the cache, restarted the console and downloaded them again, with teh same results. For 3 time.

So I'm not sure if I'm having problem with downloading them or I'm having problem with playing them.

I started to download the game from disk at 8am this morning and it still hasn't downloaded yet y do they take so long to download as ppl are paying a lot of money for games and why can't you just play games straight from disk
