Two Controller Update Issues- One not detected, one updated, but is still nagging for update

My SO and I have two Xbox Ones; for these purposes: Xbox A, and Xbox B. We have three (non-3.5mm) controllers: Controller 1 is primarily used for Xbox A, and Controllers 2 and 3 are used for Xbox B. Recently, I bought a chat pad to be used with both Xboxes.
I plugged the chatpad into Controller 1 on Xbox A, and updated the controller with no issues. Then i moved it to Xbox B, where the problems started.

Controller 2 would give me the update pop up, but would not register as being plugged in. I believe this is due to the micro-usb port on the controller being dead, as I tried it on both Xboxes, with multiple cables on each, and it would not even give me
the vibration feedback that it had been plugged in.

Controller 3 would initially not update on Xbox B, but when synced to Xbox A, updated fine and now works with the chatpad. However, when I reconnected Controller 3 to Xbox B, I still get the nag screen for the controller update every time I turn the controller
on. When I go through the controller update process, it says the controller is already up to date. I have tried a hard reset and it has not affected the update nag. Is there any way to disable it?

Anyone else have issues with TV after last update?

XBox one updated the other night and since than XBox is unable to detect my DirecTV box. Checked under Settings > TV and does not detect anything. Find it to be a huge coincidence that the XBox updates and now the HDMI cable went bad at the same time?
Going to pick up a new HDMI cable tonight to see if this is the issue, was just wondering if anyone else experienced this issue or this issue at any other point with the XBox?

DESPERATE for help !

try updating the firmware on your controller. if you haven't this could be your issue. the one controllers do not support attachments out of the box, you have to update.

Hello Ciara,

Thanks for coming to the forums with your issue. We understand the controller is not detecting the headsets. We recommend to

update the controller with the headset plugged in. Even if it shows no update available hit A on it as it will run a search for update, this will usually resync the headset/controller.

I've been having the same issue since the last update (2 weeks ago). This is NOT a Windows 10 nor Xbox ONE controller's issue, because I can perfectly use 4 controllers to control both Steam games and Xbox ONE Windows 10 App. The controllers work fine with
the app, but one I start streaming, my Xbox only detects the first controller.

So, no power-cycling nor controller's firmware update is going to fix that.

I hope they acknowledge this issue and fix it in the next Xbox ONE update.

I went to settings > system > update controller and it said "Controller is up to date". So I'm pretty sure I haven't missed any controller updates. I did a hard restart on my xbox one whenever everything was plugged in and it still isn't detecting my voice.

Any other advice?

Hi Rebecca,

Thanks for coming to the forums with your issue. We understand having issue with the controller not being recognized. A lot of people read that symbol as the mic muted, in fact it is the mic not being detected. We recommend first try and

update the controller. If it says no update available still run the check. This resyncs the controller, also do this with the headset plugged in. Try the headset in a different device also to make sure it works. Try these and let us know.
