Fixed mine go to the desktop version of twitch from your computer and deactivate your console then go to your console and sign in it will give you a new code it worked for mine

Ok i did this took the xbox off from the connections page and yeah it gives you a code but the xbox app just hangs on that page not logging in and if tou try to sign in right back to 995f9a12 error code

it signed me out and will not allow me to go back in

Sorry to hear you are running into this issue with the Twitch app. In this case you would need to reach out to Twitch support for help with this specific issue. We have no way of unlinking the twitch account as we would not have access to that information.

Go ahead and click on the following link to go over to the Twitch Support page. You should be able to open a support ticket and simply request if there is anyway to unlink your old account or if they are able to reactivate your account so that you can unlink
it.
