I bought 3D Ultra Minigolf for the Throwback Thursday 2 weeks ago, and have received the credits for it, but it has not yet added a second punch to my punchcard. I previously purchased Forza Motorsport 5, and that showed up fine.

It have punched 1 of 4 "Bonus Reward punchcard" in the page "MyMissions", Should it punch 2 of 4?

It shows "Pending Credits" in the page "MyRewards";

07/12/17 - Throwback Thursday Rewards - July Week 2 (1,000 Rewards Credits) - 1,000 Credits

06/15/17 - Throwback Thursday Rewards - June Week 3 (1,500 Rewards Credits) - 1,500 Credits

It takes about 96 hours for credits to show as pending after completing the offer, so it's too early to see the credits pending.

As stated in our FAQ (https://rewards.xbox.com/faq/):

When will I receive my Rewards Credits?

Your Rewards Credits will appear as pending within 96 hours of completion of each week’s offer or completion of the Bonus Reward punchcard.

Since your purchase was just made today, it is too early to see the Credits pending.
