It's probably just a glitch within the game; which can only be fixed with an update.

Sounds like it could be your xbox. Halo 4 probably did not work because it needs an update to play because the makers made it that way they could fix glitches and other problems so people wouldnt find glitches and exploit it. If the your copy of halo 4 works
fine on another xbox, then it is probably not due too the game but that the xbox won't let it play because it needs the update. there is not any known fix but to repair your xbox or get a new one. sorry

They've confirmed it's a glitch and your probably not banned, they're working on a fix so I've just been sitting around watching Naruto while i wait for my account to be unbanned

Somehow it works again. Probably just a temporary glitch. Did nothing in particular to fix it.

If it is a minor patch and something that is a minor change than there is no reason to announce it and just do the stealth patch and dont worry about it. The PS3 probably had something that wasnt quite so easy to fix compared to the 360 version and hence
requires a bigger patch. They obviously had tested the glitch if they told what to do when they described how to re-install the alts to fix the problem. There was probably a minor glitch with the 360 that only required a minor fix, they fixed it and told people
to re-install the alts for the fix to work.
