Hello JaxSinWrap, sorry for what's going on with your console, but by its description it really seems ago physical and not system, by the time you spoke I imagine it is out of warranty, but you have the option to use the paid repair service from Microsoft.

Then in MORE DETAILS, look to request repair, Start Online Service.

To mail it to Microsoft.

No idea what safe mode is on an Xbox but it probably doesn't fully load the XBox which means the companion app won't recognise there;s an Xbox there. Unless someone can come up with some other ideas, the only solution is to take the Xbox to a repair shop and get them to fix the HDMI port

Make sure also that the monitor supports, HDMI Displayport 2.0 as that what the Xbox One S and Xbox One X use, I believe that the standard Xbox One only uses HDMI Displayport 1.4,

Its possible that the monitor only supports HDMI Displayport 1.4,

Its possible that you're TV has HDMI Displayport 2.0, which could be the reason it works on the TV but not the monitor.

my Xbox 360 s recently froze and it messed up the video and audio. I've got an HDMI cable and I've got the video working right again but I don't have any audio what so ever any ideas?
