    Hello,

    I started receiving errors Sys_Error Fatal DB error [3, 15, #x415aea6eff0d9e9a5, 124] on my Xbox Series X. The last number can be different 124, 145 etc. It happens while playing Call of Duty Modern Warfare 2 2022 and often when my gamepad is randomly disconnected.     Often when I open my MacBook Pro (16-inch, 2019) my gamepad gets disconnected. I play above the closed notebook. Try even not to enable wi-fi on IPhone SE (2016). Under the table I have D-Link DSL-2640U modem. Xbox connected via ethernet. Maybe it will be useful.

    Additional information is needed, please tell me:

    Is this error code appearing with all your games or only with Modern Warfare II?
    Are you using an insider build / beta OS in your console? And what did you tried so far to fix this. In that way I can advise you different things or how to proceed

    About the thing with the controller, is this happening only when you connect the controller wirelessly / Bluetooth?
    Or if you try with a cable something similar happens.?

    I started receiving this error only with Modern Warfare II few days ago. I haven't played other games for some time but there wasn't any issues except the problem with the controller disconnecting. I use default OS, updating when there are new updates for Xbox or controller. All I did just reboot Xbox.

    Doesn't matter. Disconnecting happens constantly and randomly wirelessly and with the cable. Some days often some days like last 2 days it's impossible to play. I turn off Xbox, unplug it and wait for a few minutes to turn on. It connects for 20 sec and gets disconnected. When everything is fine I'm afraid to put down the gamepad because it also might be disconnected. It's related or not I don't know. It started happening after 6 months when I bought Xbox (https://answers.microsoft.com/en-us...52fe0?page=11#:~:text=Replied on May 27, 2022). That's why my Xbox is more like a piece of furniture.

    Hi, if this problem is happening only with modern warfare that doesn’t mean it’s an issue with your console.

    But we can try to isolate the problem a bit more.

    For this try:

    A. Do a factory reset for your console but keeping games and apps

    Go to settings > system > console info > reset and then select keep my games and apps

    B. Perform an offline update for your console. In that way Xbox system will be installed from scratch.

    For this you need a USB and PC to format it and transfer files provided in the link below (with detailed steps and requirements ):
