stupid ways you died or failed missions

i was doing the last mission of act 3. i was at the end where you are on top of the building. i got close to the edge and got a prompt to press be to climb down. thinking it was a ladder i had to descend, i press b and fell of the building and went splat.

i drove a bike around the corner of a small building and hit a propane tank. i didn't know what happend at first. the explosion scared me because i was not expecting it.

I have been in cover and someone comes behind me and when i try and move i cant and i get killed.

The gang hideout in the parking garage. 2 or 3 times I accidentally hopped over the ledge and fell to my death. All of them were when I was right at the end of it with the target and maybe 1 more guy left.

That, and the many times that I've not been able to get my character to "Unstick" from my cover quick enough to deal with a sudden, but otherwise easy, situation.

I was chasing a guy out of a gang hideout and when I rammed his van it flipped over. When he got out he fell inside the back of the van and got stuck. I had nothing else to do so I just kept ramming the van until his body ragdolled and it failed me.

threw down a proximity mine too close and as soon as i stepped away.. BOOOOMB!

Same here. Doing a particular mission where reinforcements keep coming, I kept taking cover right against propane tanks and getting blown to crap, took me about 5 attempt's before I realised my stupidity. Also blown myself up with steam pipes.

Was doing a gang hideout and hacked an explosive on a guy. Was watching him thinking I had him, until he threw the bomb right on me. By the time I turned to run, the explosive I hacked killed me. Complete disbelief followed by laughing too hard.
