Please try to add first your billing and shipping information through https://account.microsoft.com > Payment & billing > Address book. Also on Payment options, be sure to add your payment method. Once done, try to request a replacement order again. You could use a different device or another internet browser or try in-private or incognito mode.

Thank you for your response. I've tried all you recommended (ensured my billing and shipping address are entered under my account and checked my payment details) and then tried resubmitting an order in an incognito window and on another device. However, I still get the same issue - I'm unable to get past this screen... Do you have any other advice? Is there a phone number I can call?

The email you received also included instructions on how to ship the console and to not include any cables or accessories as it is not guaranteed that your items will return to you. The service center does not need any cables to test and repair the console.
Usually if you did send in your cables, the cables will be shipped back 10 days after the console is delivered, therefore you won't be able to connect your Xbox when you get it back. This is why it is important to keep your cables so you can hook up the Xbox
when it gets back to you.

If this is the case, you should open up a support ticket with Xbox Customer Support to see if they can follow up with your service order:

If you have tried everything on the support article troubleshooting system error E102 & E101 that was provided by the ambassador, then you will need to set up your console for repair.

You can set up the console for repair online by registering it at
https://support.microsoft.com/devices and then selecting the option that is available depending on the warranty. After you select what you want, you will be provided with an address on the nearest service center from you.
The addresses for services centers are not made publicly available, so you will need to send it to whatever address you are given from the repair process.
