Stereo Headset Adapter No Longer Works. Problem Updating

Hey there Xbox One Community. I need help with my new Xbox One Stereo Headset Adapter

When I first unboxed the adapter I plugged it into my controller and I attempted to update my controller without plugging the headphones in. The update failed. I then just plugged my headphones into the Stereo Adapter and they worked perfectly fine. I
heard sound clearly and my microphone was working in game. Keep in mind that the Update had FAILED.

This worked perfectly fine for a few hours when the xbox stated that I would need a controller update for the adapter. At this point I figured it would work and I tried it again. Once again the update failed. This time I tried it again with my other controller
and the update failed for my 2nd controller as well. I pulled out the batteries and made sure all the USB's and wires were secured.

From this point on out my Stereo Headset Adapter is PERMANENTLY Muted and PERMANENTLY has the Orange light up bar lit on the Mute button. I cannot get rid of the light up orange Mute button no matter what buttons I press. My headset no longer works and
I cannot hear or use the Microphone on my headset. The stereo headset adapter does not work for EITHER CONTROLLER.

I have reset my xbox Multiple time (5+) and have pressed the Xbox button for 5 seconds and waited 10 seconds before turning it on. I have waited and attempted to update my controller multiple times. Please note the stereo headset adapter has the Orange
Mute Bar lit up even when the headset is not plugged in. It is always on.

Any potential solutions to this issue? Please help as soon as possible. Thanks and I appreciate all the help I can get.

Stereo headset adapter issue

Brand new headset and stereo adapter will not work. Initially you could hear with headset but mic would not work. Now nothing. The mute button will not turn off. Tried hard reset on xbox one. Tried updating firmware. It says no update, controller up
to date. Tried taking batteries out of controller and reinstalling. Tried reinstalling stereo adapter and headset connection numerous times. Tried plugging headset directly into controller with no stereo adapter. Nothing still. Old turtle beach headset
plugged directly into controller works fine. This is a brand new headset and adapter right out of the box. Unbelievable.

 

How do we update the controller? NO USB Cable comes with Xbox One.

About updating controller, To use the Xbox One Stereo Headset Adapter with your Xbox One Console, you’ll need to update your Xbox One Controller with the latest software to support the Adapter.

So Titanfall controller update is done by over air, no cable, (February update) and Stereo Headset Adapter controller update you need an cable, which comes with the adapter (March update).
