Troubleshoot Xbox prepaid code issues. Currently as of the posting of your thread there is an active

Xbox Live Service Alert stating:
Affected services:

• Using prepaid cards or codes for downloadable items or Xbox Live subscriptions
  

Which may be causing your issue. It being a "Service Alert" means the correct people are aware of the issue and are working on a resolution to get the services back up and running
