Sorry to hear it. Power cycle the console by holding the power button on it for 10 seconds until it shuts down to see if that helps.

It could also be that your disc drive in the console is not reading the disc, and as those games are on Game Pass & EA Access, then chances are that is what it is. I would try power cycling the console. Instructions here

Unfortunately, Fallout 4 is not longer available on Game Pass. The message you're getting is essentially because you have the game downloaded but don't own it. With the Game Pass subscription you only have access to play them whilst they're listed on the
currently available games. It's not like games with Gold where you keep it so long as you're subscribed.

The Game Pass app is a good way to keep up to date with titles coming to and leaving the service.

Did you actually purchase the game? Since it's no longer in Game Pass you would need to have purchased the game in order to continue playing it.
