Based on the description, you have purchased the game including the DLC and it was successfully tied up with your account because you were able to play it for quite some time and suddenly disappear in your device. Let us check on this together and sort this out. Rest assured, we will do anything in our capability to assist you with your concern.

It is possible that there is other profile sign in to your device or check if your device was set as Home Console with the other user in the household. If this is not applicable, try signing out your account and sign in back to see if the game appears in your game library. Once the license of the game was digitally linked to your account it will always be tied up to your account you use to purchase.

I was told at GameStop that the Xbox 360 and PS3 season passes do not transfer to their respective next gen consoles and that you will have to repurchase the season passes if you upgrade to next gen

I have not heard anything of the sort. Your season pass will carry over to Xbox One for free but you will still need to pay $10 to upgrade to the next gen version of the game.

Yes, as long as those systems are in the same console family (Xbox 360 & Xbox One; PS3 & PS4). This was confirmed back at the MP reveal. What’s more, if you have the game on both systems, you can use the Season Pass on both — but you have to establish your
Season Pass on your current-gen console for you to be able to take it forward to the next-gen console. This is specifically an upgrade offer, so don’t wait until you get your next-gen system, get a Season Pass, and then try to bring back to the current gen
— I do not expect that to work.
