Haven't noticed this with any other game; do you know why it changes?

It has been changing since launch.

I have also noticed it for MW3, sometime it is the MW3 symbol or sometime the dude with the gun. I just happens.

First- icons are not broken in Windows Start Menu.
Was there a recent Windows update-i had this icons broken since windows 11 update.
No apps are involved into this
And about this FPS counter, Request access command just dont work.
