    Report a Problem has disappeared after latest update


    Upon attempting to report an issue with Fallout 4 achievements not registering, the Report a Problem app needed to update. After updating, the Report a Problem button is no longer available on the power menu (holding down guide button until it pops up). This persists after multiple restarts and fully power cycles that include unplugging the console.

    That's the problem, after the latest title update the BONUS mode has disappeared. I have deleted it and been forced to download the update again, but still no luck. It is a mystery. No more Nuketown for me I guess

    A lot of people are currently reporting problems downloading the latest update for Modern Warfare, so there's probably a problem with the download servers. You should be able to download successfully after the problem has been cleared up.
