1. I have not tried resetting my Xbox console at this time

2. I have not tried using a USB cable for my controller at this time

3. I have updated/continued to check for updates for my Xbox console. At this time it is currently up to date and the problem is still occurring.

I've tried leaving it disconnected from the power, when I reconnect it to the mains it turned on by itself, with the 'phantom' control in the player one position. I don't have a media remote for the Xbox, or did you mean any remote at all?
