Refund Form?

So I am having an issue with the refund form where I try to click on the Request Refund link but then I get sent to adding and removing family members and stuff like that. I don't know why it is doing this but I want to make a refund and I only have a
few days left to do it. If someone has a link to the form itself or something like that, that would be very helpful!
