I redeemed my Xbox Rewards points for a bunch of Fallout Shelter boxes, but it will only let me redeem one code.

We do not ask for phone numbers for the MyVIP Vault. Are you asking about a Xbox Live Rewards offer?

I have the same problem. I "purchased" 5 of these and only 1 has worked.

The rewards are not U.S only, you could try redeeming the reward on the
https://account.microsoft.com/rewards/redeem/ instead
