Power off and controller issues

My Xbox One has been having issues for the past few months and I haven’t seen any posts detailing the problems Im having. The first problem is that if my controller dies or I turn it off, I can’t simply turn the controller back on and start resuming whatever
I’m doing. I have to restart the Xbox in order for the controller to reconnect. Also, if I start up my Xbox and plug my headset into the controller my Xbox turns off, but if the headset is already plugged in before the Xbox is on, it’s fine. Then my Xbox always
starts with no sound and I have to unplug the headset and plug it back in. Any help is appreciated

Have this happen every so often when I power off with controller or voice (don't know about hitting the console though, don't power off that way). JUST started a little over a week ago. Controller stays on while unplugged, Kinect powers on and off at random.
Was able to eject disc, but nothing else worked other than pulling the power cord. No other issues with the console..... I are worried.

 

Can't turn XBox One on with my controller anymore.

Well Update/OS version 1408 (August 18th) update has appeared to have cleared up my controller power on/off issues with Instant-On enabled. In other words, the controller is now powering on & off with console. Now I will be monitoring the controller disconnect
issues while in-game to see if all works as should. Thanks guys! Be blessed.

This is an odd set of issues for sure. We will need to gather some info to best assist, can you provide the following:

• Does this happen with multiple headsets?
• Does it happen with multiple controllers?
• Does the same thing happen with that headset and controller on another console?
• Does the same thing happen when the controller is wired?
• Is the controller fully updated?
• Are you part of the Insider program?
• Is the console plugged into a power strip or surge protector?

• Does this happen with multiple headsets? I only have one headset; Microsoft’s basic Xbox One headset.
• Does it happen with multiple controllers? I only have one controller. However, my controller is relatively new and I didn’t have these problems on the controller before it.
• Does the same thing happen with that headset and controller on another console?
  I haven’t had the opportunity to test them on another console.
• Does the same thing happen when the controller is wired? Yes, even if the controller is plugged into the Xbox, the aforementioned problems persist.
• Is the controller fully updated? I have made sure the controller is fully updated.
• Are you part of the Insider program? No, I am not.
• Is the console plugged into a power strip or surge protector? It’s plugged into a power strip.
