Phases 2, 3, and 4 of Lord of The Rewards not triggering

So, I got the credits for Phase 1 of last month's special offer. However, I also completed phase 2 (bought the Cartman Wizard Outfit, a $3 item listed under the eligible content), and also gained a total of 7 achievements across two eligible games I purchased
to trigger phase 1 (in addition to several other achievements in an eligible title I had previously bought). Why haven't the rewards triggered? I handled both the avatar tier and the achievement tier with several days remaining. Could it be that these will
register around the same time as the rest of my monthly MyVIP deposit? If not, what iss my recourse to dispute the issue (since I have proof via billing and achievement history that I accomplished the other goals)?

Rewards for Phases 3 and 4, from the offer "Rewards Xbox University"

Hello XBox Live Rewards,

It would be very unfair for me not to receive the rewards related to Phases 3 and 4 of the offer: "Rewards Xbox University", because you counted wrong and out of time my reward for Phase 2. only recently corrected the error. And since Phases 3 and 4 depend
on the completion of Phase 2, I could only do them successfully. I hope this does not detract from my rewards from these phases, because the mistake was yours.

Lord of the Rewards not credited correctly

I completed all 4 levels of the Lord of the Rewards promo last Saturday. Today, I see only the rewards credits for level 1. For level 2 I bought the Dragon's Breath for my avatar. For level 3, I completed 5 achievements in Witcher 2. Level 3 is currently
showing 20% complete.
