"Perfect" status not displayed

However I have finished all games of the April Solitaire Collection, in the overview the "Perfect" status is not displayed. The month April itself says that I still have to play 0 challenges! How can I achieve the "Perfect" status? I have already re-installed
MS Solitaire Collection. See image below.

Received Ban but with a perfect record/no violations reported

I received a ban until October 2nd, however, when I check my account, I have a perfect record, with no issues reported. Also, when I try to do a case review, there is no "orange hammer" displayed either. PLEASE HELP!

 

"Perfect Circle Scratch"

My brother moved the 360 console while he was playing the game. He didn't accidentally move it and now there is a "perfect circle scratch'. This disc was Black ops, and no longer can I play any maps (Zombies or Multiplayer) online or offline. I tried someone
else's disc and it worked perfectly. My brother wants me to help pay for a new one because he says that because I sometimes left the game in the console, or left the game laying out outside of its cover, that this is also part of the problem. When he first
started playing the game, it worked fine. And then in the middle of the match, he moved the console. The game still loads up to the point where you are about to start playing on a map, and stops there. I think this is proof enough that it's his fault and should
pay for the new game in its entirety, but he still wants a confirmation. Please respond quickly.
