"One Successful Hack" Not Unlocking

I got the game the night it came out and some of the achievements were not unlocking for me at first, but once I restarted the game form the beginning the achievements started to unlock...save one. I have every achievements except two, one of them is "Worlds
of Hurt" (which I have not attempted yet) and the other is "One Successful Hack".

I tried everything I could think of on my own: I uninstalled my account and reinstalled it, I deleted the game and then reinstalled it, I even erased all my saved data from everywhere and still when I get to medical where you are introduced to the first
hacking attempt, I do it and nothing. So is there someone who can help me or a specialist who can navigate me to get this. It's going to drive me insane!!!!

Why can't I play online with a silver account?

If you know which user did the hack, report them through your console for the Enforcement team to investigate.

If you end up in a modded CoD game you should always leave the game immediately to avoid any risk of future bans. I could not say if there would be a risk to your account as that would be up to the enforcement team to decide should you be reported to them.

Yes, find the player in the list of recent profiles you played with, and then report that.

Also, whenever you find you're in a hacked game, immediately quit hard out of the game.

On the Xbox 360, press Xbox button then Y to quit to the dashboard, and on the Xbox One press the Xbox button, press the menu button on the main game tile, and choose to Quit the game.

Otherwise the game will upload the statistics to the Call of Duty servers, messing up your profile and statistics.

And the longer you stay on a hacked game, the higher the probability, that Xbox will think you're willfully playing on a hacked game, increasing the chance you get a suspension.
