One Chat Headset Problem?

I'm having a problem with my One Chat Headset. I'm now on my second one after I exchanged the first at Best Buy, but am running in to the same problem, so must be something I'm doing/not doing correctly. The headset works fine and dandy for the first week
or so on Live, and then randomly can no longer registers my voice nor can pickup audio from other players. If I plug the headset in I can still hear the volume beep when I turn it up to the maximum level, and the orange light still turns on when I mute it,
but I can hear any other audio/it doesn't pick up my voice. Any ideas on how to fix this hopefully simple problem? Thanks!

Chat headset transformed into malfunctioning Stereo Headset!!!!

Hi,

It seems that my controller (chat headset) was updated with stereo headset firmware and I am only using a chat headset...

This has resulted in game audio coming through my chat headset and I am unable to chat unless I hold the mute button down!

XBox one online support chat is also offline - Using my PC now.

 

When this happens, can you back up to the Home screen and select your profile's display picture (using your control) and hit "Choose this person." This seems to often rectify this error.
