When did you register? Per the fine print, if you registered before August 24, the codes would have been sent on or around August 29. But if you registered between August 21 and September 2, codes will go out around the 6th.

Verify your info, especially the zip code where the card is registered. I know that I've mixed up that zip code with previous registered locations, such as before I moved.

Is it a debit card linked to your bank, or a gift card that works as a debit card? You may need to register it before using it.

If you forgot the code, you're going to need to contact customer support 1-800-4MY-XBOX. They have a reset code they can get from their systems after registering the xbox to an account. If you already have your xbox registered, it's a quick process to get
the code.
