No sound (periodically) in headsets and controller disconnecting/freezing

Hi - I'm hoping someone can help me figure this out.

The past week, I've been having trouble with my Xbox One.

To start, I've been having issues with my headset (Turtle Beach XO Seven) not picking up ANY sounds (gameplay, dashboard etc) and other times it will, but when the headset is removed from the controller a few times, or sometimes on startup, I can't hear
any sounds at all; (I have tried different headsets and different controllers and the same thing happens.)

Second, my controller(s) will freeze or lose connectivity, and I can't turn them back on and reconnect to the Xbox One (I have to shut down the system.)

Not sure if it has anything to do with it, but my Xbox One is a Day One Edition.

I'm hoping someone has experienced the same issue and was able to resolve it; I love my consoles so much and would hate to have to repair it / get a new one (but if I have to do it, I have to do it.)

Any input or feedback would be greatly appreciated.

Xbox update

The controller update has to do with the wireless chat audio adaptor and the controller itself. Fixes certain issues like disconnects or controller freezing as well as static sounds heard in the headset when using Dolby Surround setting selected under Bitframe
for audio.

I'm going to check it again tonight since it appears I got the update, but no indication.

Xbox One Stereo Headset Adapter

I bought a stereo headset adapter and use it in conjunction with my studio beats. Everything was working fine one day, next day it doesn't work at all. No sound no matter what I do. I know the headset work as I've tried it with my laptop.

I have tried:

Connecting different headsets.

Resetting Xbox, disconnecting and reconnecting adapter and headset.

Updating controller via usb, with and without battery pack in. With and without headset adapter connected. With 2 different headsets connected.

Nothing seems to be working.

Any help would be appreciated.

So, I went ahead and reset my Xbox One (factory reset) and it seems to have fixed my issues; it also seems to be running a lot smoother and faster too - not sure why, but I am not complaining!

When you plug the headset in does it give you a little menu on pop out window to control the volume of chat and game? If so the sound bar is disconnecting because it is trying to use the headset instead.

I've been having the same problem since I got my new headset. My headset makes a sound as if its been disconnected then my controller turns off. It wasn't happening before I got my headset today.

remember these consoles are not able to handle the power of headset at the same time, my console doesn't freeze till I disconnect the wireless receiver of the headset, which means it freezes when I connect the receiver during the black ops 2 game play and
freezes when I disconnect the wireless receiver.
