I think there is a misunderstanding "Headphone Stereo Jack plug
connected to the headphones right?

Also downloaded [Xbox Accessories App] which was not Installed

No, I do not have a "an adaptor" I don't know which "adaptor" you mean? (why would I want an "adaptor" when the whole purpose/point was to buy this
NEW Xbox One controller - connected through Bluetooth! without any adaptors/dongles...etc.)

I don't know what I did but noticed controller was paired but
not connected?

Now its connected and works through [Xbox Console Companion App] and if I launch games from
side menu but still having problems/issue with [Xbox Beta PC App] Freezes.

Now have Audio issue/problem...start new post.

Pull out the batteries in the controller that works, and try pairing the new one before pairing the original. If that doesn't work, it's likely you have a faulty controller.
