Never received my reward for January Xbox Quest

It is now March and I have not received my reward for January Xbox Quest. I have checked my "Messages from Xbox Live" everyday and haven't gotten it. I'm in the United States and I for sure made the requirements and had my Gold on the entire time.

February is over and I didn't received my January Xbox Quest reward

I did all the requirements stated in the banner, and never received my Xbox January quest reward.

Thanks for helping me with January's, but now I haven't received February's either. Could help me again? I know for I made the requirements and didn't get it once again.
