And I let my cousin borrow my account on her Xbox one but when I got my account back everything was changed my name and gamertag my bio and I have no clue what the problem is

Have you made sure that you are logging into the correct account? Make sure that you have the same gamertag, if by chance you are in the incorrect account you will need to figure out what the email was to the other account so that you can try and login,

A code can only be redeemed once, please check your Ready to Install section in My Games & Apps on the console to check if the game is ready to install.

Had the same thing happen to me and its been over 2 weeks and every time I try to put the code in again, its just says that its already been redeemed.
