Please don't list private info on the forums.

If the controller was purchased 2 weeks ago, return it to the place of purchase.

if you bought it through the Microsoft/Xbox store go to https://support.xbox.com/en-US/xbox-one/controllers/wireless-controller-solution
and follow the guide selecting the appropriate answers.

this will either fix the controller or take you to the form to request repair or replacement

Hello, I would try to contact a representative over the phone. Just an FYI phone hold times are VERY long, but it will be someone there to answer. When turning things in for repair it states what to send and what not to send. As you found out all those
other accessories are never sent back.

Contact Microsoft. After a few hours on the phone you'll pay out of pocket to send them your broken controller. A month of two down the road you will call because they never sent you anything. After hours on the phone they may recover your repair order.
A month later they will send you another broken controller. Good luck.
