You will need to log on to http://enforcement.xbox.com, check your Enforcement history, and submit a case review as your appeal (if eligible).

The only way to get a free name change is if it is seen as offensive by the Xbox Live Policy Enforcement Team. While some people have tried to "play" the system to get out of paying MS points, doing so can have a negative result on your account. My suggestion
is to save your money and change it the right way.

I'm pretty sure you can also buy them from the Xbox itself.

Alternatively you might want to save up some moeny, otherwise you won't be allowed on your Xbox.

The above is somewhat true, I think it's possible your new Gamertag actually breaks the Code of Conduct (implying the ability to hack games/alter the LIVE service) but if you have a forced Gamertag change for this reason the Enforcement action will always
be visible on your account, without the context of it having been someone else who made the offending tag. This can throw up other problems trying to access other things (for instance, you would never be allowed to apply to become an Ambassador, because of
the Enforcement action).

You can contact Phone Support and see if they can do anything at all, although it is unlikely.

So you can either wait for the almost-inevitable Enforcement action, or get ahead and make the name change yourself, although you would be charged for that one.
