Mic icon appears with line through on home page

Hi, my sons Xbox One S now loads and the mic icon next to the battery icon has a line through it. This now stops him being able to talk to his friends which he is trying to do through Fortnite. Ive reset the controllers last night and eventually it worked
and he could chat with them but this morning the same issue has come back. Ive tried going into settings but nothing seems to work. Can someone explain why the mic icon has a line through it and how do i resolve this? Thanks

Setting up a headset adapter. This helps if the 3.5mm socket is faulty and also

Having issues with my sons headset not working. On the home screen there is a microphone with line through it, how do i change this? I tried going into settings but i could not figure out where to look for the microphone setting

Be careful spending money on new hardware only for the same issue to reappear in a matter of days. This has happened to me. This is a known issue / bug with some consoles that microsoft needs to fix. This is their issue.

I dont have one anymore, I dont understand how/why its now appeared as none of his friends seem to be suffering with the same problem.
