    Played gears of war 4 insane campaign completed unlocked inconceivable difficulty but me and my partner didn't receive the two achievements for hardcore and insane?

    Casual of course, I'm just looking to get the 4 Player Campaign Medal, I can beat the Campaign on Insane myself, I've already done that on Gears 1 & 2 : )
